Patient safety after launch

Pharmacovigilance

Patient safety does not end at the factory gate. If you notice an adverse reaction or a quality problem with one of our products, your report is an essential part of the drug-safety system.

An emergency?

If you are experiencing severe or life-threatening symptoms, go immediately to the nearest medical centre or call emergency services. This page is not a channel for medical emergencies.

Who can report?

A report needs no professional standing. It only needs to reach us.

  • Patients & carers

Do not wait for a professional opinion before reporting. Describe what happened in your own words; the scientific assessment is ours to make.

  • Healthcare professionals

Physicians, pharmacists and nurses: your report carries valuable clinical detail on dose, timing and concomitant medicines.

  • Pharmacies & distributors

Packaging, storage and supply-chain complaints arrive here too, and follow exactly the same route.

What should be reported?

  • Any unexpected reaction after taking the product, however minor it seems.
  • Absence of the expected therapeutic effect.
  • A dosing or administration error.
  • A visible defect in the pack, appearance, colour or odour.
  • Use during pregnancy or breastfeeding and its outcome.

What information do we need?

  1. 1Product trade name, batch number and expiry date.
  2. 2A description of the reaction, its onset date and duration.
  3. 3Patient age and sex, and other medicines being taken.
  4. 4Contact details for follow-up if needed.

These fields let us tie the report to a specific batch. Leave them blank if you do not have them.

A quality complaint rather than a reaction?

If the problem is with the pack, appearance, colour or odour and no reaction accompanied it, send it through the same form and choose “Product complaint” as the subject.

Check the package leaflet first

Every product page carries its full package leaflet, covering known side effects, storage conditions and drug interactions.

What happens after you report?

Four steps every report passes through, however minor it seems.

  1. 01

    Logging

The report is entered into the pharmacovigilance register on arrival and given an internal reference number.

  2. 02

    Clarification

We may contact you to complete a missing detail: the batch number, the onset date, or concomitant medicines.

  3. 03

    Scientific assessment

The team reviews the batch record and its retained sample, and assesses the possible causal relationship between product and reaction.

  4. 04

    Action & follow-up

The appropriate action is taken — from updating product information to withdrawing a batch — and we come back to you with the outcome if you ask.

Report form

Fill in only what you know. Missing fields do not block a report, and an incomplete report beats one never sent.

Product details

These fields let us tie the report to a specific batch. Leave them blank if you do not have them.

At least 20 characters. The clearer the detail, the more precise the answer.
